Review and compile the data from your community and family assessments. Complete the identifying data section and Environmental sections of the short form by week two. The remaining sections can be counted as clinical and submitted by week 5. Use the attached forms to conduct home visits and create the data. These need to be completed in your clinical (once the family contract has been approved) by week five of the course.

Complete a Windshield Survey for the community where this family lives. Take 2 days to complete this survey driving around one day during the daytime hours and a second day in the nighttime hours (after dark). See the resources, your Nies text, and announcement regarding how to do a Windshield Survey.

2. Ensure you have included an explanation of issues that apply to your family’s community on the short form based on the identifying data and environmental data.

3. Format your genogram . A genogram (pronounced: jen-uh-gram) is a graphic representation of a family tree that displays detailed data on relationships among individuals. It goes beyond a traditional family tree by allowing the user to analyze hereditary patterns and psychological factors that punctuate relationships.

Create a genogram asking the family what their predominate medical problem is and then do a genogram tracing that problem.

4. Create an ecomap (An Ecomap is a diagram that shows the social and personal relationships of an individual with his or her environment. It is often used in counseling by social workers or nurses. Ecomaps were developed in 1975 by Dr. Ann Hartman who is also credited with creating the genogram. Use the link below (copy and place in your web browser) to find a template to use for an ecomap:

5. Select one theory relevant to family/community health (a non-nursing theory) and one non-nursing theory (see this week’s study guide for suggestions) that will help the family adjust their behavior. Summarize the key points of each.

· Explain the specific application to family/community health or patient behavior.

· Address the strengths and limitations of each.

· Explain how the theory integrates the nursing process and is applicable at each level of prevention.

6. Include at least one peer-reviewed and one evidence-based reference in an APA-formatted reference page of at least 3 references.

Format your: genogram and ecomap and theory as:

· a 4-8-slide power point presentation with speakers notes

Submit your assignment.

· Friedman’s Assessment sections as listed above

· Windshield Survey as a 350-500 word summary OR 4-6 slide power point with speaker notes

· Genogram, ecomap and theory presentation

I will send you the Friedman Family Assessment Model. The first part of the model asking to fill the info of the family please ignore it I will do it. The family I am working with is consists of 3 members, husband, wife and their grand mother. HTN , Obesity and Diabetes these are the health issues they are dealing with. Thank you
